OBSERVATION: The designated operator inspection reports for the previous 36 months were not found on site. REGULATION GUIDANCE: Designated operator inspection reports prior to October 1, 2018 shall be retained on site for twelve months. Designated operator inspection reports on or after October 1, 2018 shall be retained on site for thirty-six months. CORRECTIVE ACTION: Locate and ensure that copies of the previous thirty-six months of designated operator inspection reports are maintained on site. Submit copies of the missing reports to the EHD. This is a minor violation. 211 CCR 2715(c)(2) No employee present that were trained in the operation and maintenance of the UST system by a DO. OBSERVATION: The designated operator employee training for Dave Singh was not found on site. REGULATION GUIDANCE: The designated operator shall train facility employees in the proper operation and maintenance of the UST system at least once every 12 months. The training shall be conducted before the individual performs the duties of a facility employees. The training shall include, but is not limited to: 1. Operation of the UST system in a manner consistent with the facility's best management practices 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan 3. Employee's role with regard to spills and overfills as specified in the facility's response plan 4. Name of the contact person(s)for emergencies and monitoring equipment alarms Initial training shall also include a site-specific practical demonstration that provides both sharing information and evaluating knowledge for responding to emergencies, leak detection, monitoring alarms, spills and overfills. At least one of the facility employees present during operating hours shall have current training. CORRECTIVE ACTION: Ensure that employees have been trained by the designated operator, maintain the list on site, and submit a copy of the training records to the EHD. This is a minor violation. 215 CCR 2712(1) Failure to retain a copy of the permit to operate at the facility. OBSERVATION: A copy of the UST operating permit was not found on site. REGULATION GUIDANCE: A copy of the UST operating permit and all attachments and conditions shall be retained at the facility. CORRECTIVE ACTION: Immediately obtain a copy of the permit and retain it on site. This is a minor violation.
