Scotts Ave. <br /> Stockton, CA <br /> San Joaquin County Environmental Health Department (EHD) has reviewed the <br /> "Quarterly Report— Fourth Quarter 2003" (report) dated April 1, 2004, prepared <br /> by Advanced GeoEnviron mental, Inc. for the above referenced site. <br /> The report documents additional investigation of the soil and groundwater <br /> contamination from the former underground storage tanks by installation of three <br /> monitoring wells and a soil boring. It recommends initiation of a quarterly <br /> monitoring program, performance of a sensitive receptor survey and additional <br /> assessment to further define the lateral and vertical extent of groundwater <br /> contamination. <br /> Quarterly reports are to be submitted to EHD until the investigation and cleanup <br /> of the site are complete. The reports are to include the results of all corrective <br /> actions conducted to determine the nature and extent of the soil and ground <br /> water contamination (ground water sample results from monitoring wells), <br /> proposed cleanup actions and a time schedule for implementing the actions. <br /> After additional sampling and analysis of the ground water from the monitoring <br /> wells as well as determination of ground water flow, further investigation of the <br /> lateral and vertical extent of soil and ground water contamination should be <br /> proposed. <br /> EHD previously agreed with the recommendation to perform a sensitive receptor <br /> survey and requested the results be submitted on March 17, 2003. A survey to <br /> identify all wells within 2000 radial feet of the site must be conducted. The <br /> results of the well survey that includes a map showing the location of the <br /> identified wells, the addresses where the wells are located and any known <br /> construction details of the wells must be submitted to EHD by June 1, 2004. <br /> The report included elevation surveys completed by Morrow Surveying for <br /> monitoring wells MW1, MW2 and MW3.
