PHS/EHD has a contract with California State Water Resources Control Board to <br /> conduct an underground storage tank corrective action program. <br /> The above referenced site had a documented release of petroleum hydrocarbon and has completed a soil <br /> and groundwater investigation. During the course of the investigation, three monitoring wells (MW) were <br /> installed. One of the three MW was installed on your property at 1022 East Hazelton Avenue (APN 151- <br /> 280-36) on January 14, 1991. <br /> i <br /> On January 22,1996, ERM-West under contract with Atchison, Topeka, and Santa Fe Railway Company <br /> arrived on site to properly destroy the three MW as'required by PHS/EHD. Due to wet field conditions and <br /> rainy weather, you denied full access to the property and the MW#3 at 1022 East Hazelton Avenue was <br /> not adequately destroyed.
