PHS/EHD has a contract with California State Water Resources Control Board to
conduct an underground storage tank corrective action program.
The above referenced site had a documented release of petroleum hydrocarbon and has completed a soil
and groundwater investigation. During the course of the investigation, three monitoring wells (MW) were
installed. One of the three MW was installed on your property at 1022 East Hazelton Avenue (APN 151-
280-36) on January 14, 1991.
i
On January 22,1996, ERM-West under contract with Atchison, Topeka, and Santa Fe Railway Company
arrived on site to properly destroy the three MW as'required by PHS/EHD. Due to wet field conditions and
rainy weather, you denied full access to the property and the MW#3 at 1022 East Hazelton Avenue was
not adequately destroyed. ERM-West has attempted to gain access to your property to complete the
destruction but have not been successful.
PHS/EHD has made record of the improperly abandoned well at 1022 East Hazelton Avenue and would
appreciate your cooperation in this matter.
If you need clarification or have questions regarding this letter, contact Harlin Knoll at (209) 468-3442.
Donna Heran, RENS, Director
Environmental Health Division
i
Harlin r in boll Senior MargareLagorio, REH
Site Mitigation Unit Lead Senior
HK/1033mwde
i
cc: CVRWQCB, Elizabeth Thayer
cc: Santa Fe Railway
cc: ERM-West
A Division of San Joaquin County Health Care Services
