PHS/EHD has a contract with California State Water Resources Control Board to
conduct an underground storage tank corrective action program. The above referenced site was placed
in the PHS/EHD Local Oversight Program (LOP) in July 1989.
San Joaquin County Public Health Services, Environmental Health Division (PHS/EHD) has reviewed the
above referenced site for closure. Based on information which is currently and actually known to
PHS/EHD, this agency has determined that no further corrective action at this site is required.
The three monitoring wells at the site must be destroyed by a C-57 licensed driller under permit and
inspection of PHS/EHD. Please submit a permit application and $60 permit fee. Once the permit has
been issued the destruction activity can be scheduled.
After the monitoring wells are destroyed PHS/EHD will issue the final closure letter and the site will be
removed from the Local Oversight Program.
If you have any questions contact Harlin Knoll at (209) 468-3442.
Donna Heran, REHS, Director
Environmental Health Division
Harlin Knoll, REHS Diane M.
