I never made it back to the <br /> office last week and never saw this email from you. <br /> Regarding the permiting of the NON NPL site, YES...a permit is needed. <br /> we file our sites by ADDRESS. please provide it so I can direct you to the inspector assigned <br /> to the site. <br /> generally, boring permits are needed PER parcel and PER drilling contractor. If the property is <br /> NOT under the ownership of the applicant of the permit, an ACCESS AGREEMENT is needed <br /> from the landowner before we can issue the permit. IF the drilling in on a public right of way, <br /> then an ENCROACHMENT PERMIT is needed from the municipality with responsibility for the <br /> ROW. Some agencies also require a REVOCABLE PERMIT if a well/traffic box is set. <br /> A Master File Record (MFR) is to be provided with the permit application. I've attached one <br /> here for you along with a UNIT IV- Site Mitigation Permit Application and its page two, the <br /> "Licensed Contractor Declaration" (LCD). We issue permits to Licensed DRILLING <br /> contractors, not consultants. Please have the driller fill out and PDF, fax, or mail the LCD to us. <br /> permits are issued to drillers under a lead agency work plan approval.
