s , ` e <br /> Request for Proposal <br /> August 13, 1987 <br /> Page 2 <br /> INITIAL ASSESSMENT OF SITE CONTAMINATION FROM LEAKING <br /> UNDERGROUND STORAGE TANK: 110 WEST SONORA AVENUE <br /> entitled "Consultant Reports for Assessing Ground Water <br /> Problems at Sites Contaminated by Leaking Underground Storage <br /> Tanks (LUSTs) " dated March 24, 1987 . <br /> NOTE: If your firm submitted a proposal for this project <br /> based on the RFP dated July 7', 1987, it is acceptable. Please <br /> notify us by letter that you want us to use the previous <br /> proposal. This RFP is being reissued due to the scope of the <br /> project. Council Policy requires Council authorization for <br /> x <br /> projects exceeding $20, 000. <br /> PROPOSAL EVALUATION <br /> Proposals will be evaluated based on completeness, conformance <br /> to Regional Water Quality Control Board regulations and costs. <br /> Due to the nature of the project accurate costing is not <br /> possible but present a budget that realistically addresses the <br /> cost of completing the evaluation and development of a remedial <br /> plan. The proposal must list the personnel that will work on <br /> the project and their qualifications. A project manager must <br /> be designated. This person will be named in the actual <br /> contract and cannot be changed without prior written approval <br /> from the City once the contract is executed. <br /> Proposals, 3 copies, shall be submitted to the City of Stockton, <br /> Department of Public Works, by 5: 00 p.m. , August 28 , 1987. The <br /> package must be received by this date and time, packages posted <br /> but not delivered will not be accepted. <br /> AFFIRMATIVE ACTION <br /> The City of Stockton has adopted an Affirmative Action Policy <br /> which governs all firms contracting with the City. The selected <br /> firm will be required to comply with the attached Policy. <br /> HARRY W.
