To date, this office has <br /> received no work plan. <br /> Please submit the previously requested work plan. Include in the work plan a site <br /> specific Health and Safety Plan and a well survey that identifies all municipal, domestic. <br /> agricultural, industrial and other wells located within a 2000-foot radius of the site.
