The elinin-of-custody form includes Kleinfelder's name, address and <br /> telephoto number, tLc employee nurnber of the individual who perfonned the sampling, the <br /> sample numbers, the date and time the samples were collected, the number of containers each <br /> santplo oecapics, the sample matrix (soil or water) and the analyses for which the samples are <br /> beiu; submitted, if any.
