WM - WASTE MANAGEMENT SERVICE PROVIDER <br /> PICK-UP AND REQUIRED DOCUMENTS <br /> Some states require scheduled pickups. In most states, stores will initiate a pickup only when a bin is <br /> full. <br /> For Hazardous Waste pick-up: Complete the Non-Saleable Checklist(s) and fax to Waste <br /> Management at 855.254.2246. If you do not receive a fax confirmation that the fax was received, call <br /> Waste Management at 1-866-819-1926. Stores should plan to have all hazardous waste removed, <br /> not just the bin that is full. Ensure the Hazardous Waste Call/Pick Up Log is complete. <br /> For Universal Waste pick-up: Complete the Universal Waste Pick-up Request Form and fax to <br /> Waste Management at 855.254.2246. If you do not receive a fax confirmation that the fax was <br /> received, call Waste Management at 1-866-819-1926. Stores should plan to have all hazardous waste <br /> removed, not just the bin that is full. Ensure the Hazardous Waste Call/Pick Up Log is complete. <br /> Waste Management will provide this information to the Transporter. Once the Transporter has the <br /> information, the pick-up is scheduled and the Store Team Leader will receive confirmation of the pick- <br /> up date. <br /> The Transporter will complete a "Hazardous Waste Manifest"; this is basically the Bill of Lading (BOL) <br /> showing the removal of the waste. This manifest will be updated with the items or the weight that the <br /> Transporter is removing. The Transporter will leave one copy of this manifest with the Store Leader. <br /> The Store Leader must keep this for their records. This document must be retained, along with all <br /> of the Non-Saleable Hazardous Material Checklists, and placed behind the Vendor Log tab in the <br /> Program Binder in the Manager's Office. <br /> ernmwaiw.�rvsa.ww..nee.w. oesrxereo.ncium.00rmxnwx xnnnFxeamxeo <br /> A final manifest will be sent to your store. It must be stapled to the original manifest and Non-Saleable <br /> Hazardous Material Checklists behind the Vendor Log tab.
