With Brown&Caldwell now gone,the current billing party is unidentified. In order to process boring/well permits,the billing party MUST be on record with our office. Brown&Caldwell was the last consultant and left without settling their account. Currently I am working with them to pay off the balance and I expect the balance this week. Until then,our department does not allow me to accrue additional charges against this site without a billing party. In order for you,on behalf of your client Georgia-Pacific,to continue their investigation under the deadlines established by the CVRWQCB lead,the billing-responsible party must be identified with PHS-EHD. The enclosed form is to be completed and returned with a'pre-payment'of$267.00(3 hours @$89 per)to start a new account number so I may start processing your most current work phase under consideration by the CVRWQCB. If NEITHER the'property owner'or'business information'noted on the MFR is NOT the entity to receive the billing, please identify under"THIRD PARTY BILLING INFO"(close to the bottom of the MFR form)the name of the billing party. If,like Brown and Caldwell,AME wishes to be the billing party,enter your information here. You will receive our billing every thirty days as fees for services are accrued(permit processing,faxes,and field-grout inspections). Once the$267 is received and an account started,I can begin to process the permits. As an additional note,if you DO list AME as the billing party,I strongly recommend you check with me prior to settling your account with GP if you no longer act as their consultant. Brown and Caldwell didn't and their delinquent account is causing delays in processing your permit for the continuing investigation by holding up issuing new permits.
