SAN J 0 A Q U 1 N Environmental Health Department <br /> U ; i y 1 Iii AMENDED <br /> 1/30/2020 <br /> Small Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> AFFORDABLE AUTO REPAIR AND SMOG 1 121 S Wilson Way, Stockton January 28, 2020 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 113 HSC 25160.2(b)(3) Failed to keep copies of consolidated manifesting receipts for three years. <br /> OBSERVATION: Copies of consolidated manifest receipts from the following dates for the disposal of the following <br /> hazardous wastes were not found on site during the inspection: <br /> -3/29/2017 for 200 gallons of used oil <br /> -3/29/2017 for 70 gallons of used antifreeze <br /> -7/14/2017 for 250 gallons of used oil <br /> -7/14/2017 for 100 gallons of used antifreeze <br /> - 10/30/2017 for 240 gallons of used oil <br /> - 10/30/2017 for 80 gallons of used antifreeze <br /> -7/3/2018 for 185 gallons of used oil <br /> - 10/16/2018 for 95 gallons of used oil <br /> - 10/16/2018 for 60 gallons of used antifreeze <br /> REGULATION GUIDANCE: The generator shall retain each consolidated manifest receipt for at least three years. <br /> This period of retention is extended automatically during the course of any unresolved enforcement action <br /> regarding the regulated activity or as requested by the department or a certified unified program agency. <br /> CORRECTIVE ACTION: Immediately locate a copy of all consolidated manifest receipts from the dates listed <br /> above and maintain them on site. Provide a copy of the consolidated manifest receipts and a corrective action <br /> statement to the San Joaquin County Environmental Health Department. <br /> This is a minor violation. <br /> 115 CCR 66266.81(a)(4)(B)Failed to retain copy of manifest or bill of lading for spent lead acid batteries for 3 years. <br /> OBSERVATION: Bills of lading for the recycling of lead acid batteries from 1/28/2017 through 1/28/2020 were not <br /> found on site.According to Mashin Mohammad, business owner,the facility takes approximately 1-2 spent lead <br /> acid batteries every two weeks to Carquest or Autozone. <br /> REGULATION GUIDANCE: <br /> (A)The generator shall use either the manifest or a bill of lading which fulfills the requirements of Title 13 CCR <br /> section 1161,to record the shipment of spent lead-acid storage batteries to a person who stores the batteries or <br /> who uses, reuses, recycles or reclaims the batteries or their components. <br /> (B)The generator shall retain, at the generator's place of business for at least three years, a legible copy of each <br /> manifest or bill of lading which identifies spent lead-acid storage batteries shipped to a person who stores the <br /> batteries or who uses, reuses, recycles or reclaims the batteries or their components. <br /> CORRECTIVE ACTION: Provide copies of battery core bill of lading records from 1/28/2017 through 1/28/2020 <br /> and a corrective action statement to the San Joaquin County Environmental Health Department within 30 days. <br /> This is a Class II violation. <br /> FA0022837 PR0539930 SCO01 01/28/2020 <br /> EHD 22-01 Rev.9/20/2019 Page 7 of 17 Small Quantity Hazardous Waste Generator OIR <br /> 1868 E.
