The following is an itemized list of aboveground petroleum storage act violations that have not been addressed for DAMERON HOSPITAL as of June 02, 2020. Violation#611- Plan failed to include a description of potential equipment failure or provide equivalence. Potential equipment failure and the resulting spill for the 275 gallon day tank, 150 gallon day tanks and the 100 gallon day tank were not addressed in the Spill Prevention, Control, and Countermeasure(SPCC) Plan. The SPCC Plan must include a prediction of the direction, rate of flow, and total quantity of oil which could be discharged from the facility as a result of major equipment failure. In addition to the minimal prevention standards listed under this section, include in your Plan a complete discussion of conformance with the applicable requirements and other effective discharge prevention and containment procedures listed in this part or any applicable more stringent State rules, regulations, and guidelines.
