Title 22• <br />qvironmental Health Standards—Hazardous Waste <br />§66265.19. Construction Quality Assurance Program. <br />(a) CQA program. (1) A construction quality assurance (CQA) pro- <br />gram is required for all surface impoundment, waste pile, an([ landfill <br />units that are required tocomply with sections 66265.221 (a). 66265.254, <br />and 66265.301(a). The program shall ensure that the constructed unit <br />meets or exceeds all design criteria and specifications in the permit. the <br />program shall be developed and implemented under the direction of a <br />CQA officer who is a California state registered professional Civil engi- <br />neer. <br />(2) The CQA program shall address the following physical conmpo- <br />nents, where applicable: <br />(A) Foundations: <br />(B) Dikes: <br />(C) Low -permeability soil liners: <br />(D) Geomembrane (flexible membrane liners): <br />(E) Leachate collection and removal systems and leak detection sys- <br />tems: and <br />(F) Final cover systems. <br />(b) Written CQA plan. Before construction begins on a unit subject to <br />the CQA program under subsection (a) of this section, the owner or oper- <br />ator shall develop a written CQA plan. The plan shall identify steps that <br />will be used to monitor and document the quality of materials and dnecon- <br />dition and manner of their installation. The CQA plan shall include: <br />(1) Identification of applicable units. and a description of how they will <br />be constructed. <br />(2) Identification of key personnel in the development and implemen- <br />tation of the CQA plan, and CQA officer qualifications. <br />(3) A description of inspection and sampling activities for al l unit com- <br />ponents identified in subsection (a)(2) of this section. including observa- <br />tions and tests that will he used before, during, and after construction to <br />ensure that the construction materials and the installed unit components <br />meet the design specifications. The description shall cover: Sampling <br />size and locations: frequency of testing: data evaluation procedures: ac- <br />ceptance and rejection criteria for construction materials: plans for im- <br />plementing corrective measures: and data or other information to be re- <br />corded and retained in the operating record under section 66265.73. <br />(c) Contents of program. (1)'Mc CQA program shall include observa- <br />tions, inspections, tests. and measurements sufficient to ensure: <br />(A) Structural stabilityand integrityof all components of theunit iden- <br />tified in subsection (a)(2) of this section: <br />(B) Proper construction of all components of the liners. leachate col- <br />lection and removal system. leak detection system. and final cover sys- <br />tem, according to permit specifications and good engineering practices, <br />and proper installation of all components (e.g.. pipes) accordin g to design <br />specifications: <br />(C) Conformity of all materials used with design and other material <br />specifications under sections 66264.221. 66264.251, and 66264.301 of <br />this chapter. <br />(2) The CQA program shall include test fills for compacted soil liners. <br />using the same compaction methods as in the full-scale unit. to ensure <br />that the liners are constructed to meet the hydraulic conductivity require- <br />ments of sections 66264.221(c)(1). 66264.251(c)(1), and <br />66264.301(c)(1) of this chapter in the field. Compliance with the hydrau- <br />lic conductivity requirements shall be verified byusing in-situ testingon <br />the constructed test fill. The test fill requirement is waived where data are <br />sufficient to show that a constructed soil liner meets the hydraulic con- <br />ductivity requirements of section sections 66264.221(c)(1). <br />66264.251(c)(1), and 66264.301(c)(1) of this chapter in the field. <br />(d) Certification. The owner or operator of units subject to section <br />66265.19 shall submit to the Department by certified mail or hand deliv- <br />ery, at least 30 days prior to receiving waste, a certification signed by the <br />CQA officer that the CQA plan has been successfu lly carried out and drat <br />the unit meets the requirements of sections 66265.221(a). 66265.254, o r <br />66265.301(a). The owner or operator may receive waste in the unit after <br />30 days from the Department receipt of the CQA certification unless die <br />§ 66265.32 <br />Department determines in writing that the construction isnot acceptable. <br />or extends the review period for a maximum of 30 more day s, or seeks <br />additional information from the owner or operator during this period. <br />Documentation Supporting the CQA officer's certification shall he fur- <br />nished to the Department upon request. <br />No tr: Authority cited: Sections 25150 and 2.5159. Seismic and Precipitation Design Standards. <br />(a) Except as provided by section 66265.192, facilities subject to this <br />chapter and all cover systems and drainage control systems required by <br />this chapter shall be designed to function without failure when subjected <br />to capacity. hydrostatic and hydrodynamic loads resulting from a <br />24-hour probable maximum precipitation storm. <br />(b) Ihe following shall he designed. constructed and maintained to <br />withstand the maximum credible earthquake without the level of public <br />health and environmental protection afforded by the original design be- <br />ing decreased- <br />(1) <br />ecreased <br />(1) all surface impoundments. waste piles. landfills and land treatment <br />facilities subject to this chapter: and <br />(2) all covers and cover systems required by this chapter and all con- <br />tainnnent and control features which will remain after closure at perma- <br />nent hazardous waste disposal areas. <br />No,m Authority cited: Sections 208 and 25150. Required Equipment. <br />All facilities shall be equipped with the following. unless it can be <br />demonstrated to the Department that none of the hazards posed by waste <br />handled at the facility could require a particular kind of equipment speci- <br />fied below: <br />(a) an internal communi cations or alarm system capable of providing <br />immediate emergency instruction (voice or signal) to facility personnel: <br />(b) a device, such as a telephone (immediately available at the scene <br />m <br />of operations) or a hand-held two-way radio. capable of sumoning <br />emergency assistance front local police departments. fire departments. <br />or State or local emergency response teams: <br />(c) portable fire extinguishers. fire control equipment (including spe- <br />cial extinguishing equipment. such as that using foam. inert gas. or dry <br />chemicals),s�pilllccontrol equipment, and decontamination equipment: <br />Page 759.54(i) lt�z;n�«.N�.. z ;� a <br />
