—2— May 13,2004 27 &30 Hazardous waste manifest and receipts must be kept a minimum of 3 years. Hazardous waste manifests and receipts could not be provided from 2002 during the inspection. This information is incorrect. There was no "manifested" waste removed from this site during 2002. All of the appropriate documentation, for the items that were removed,was in the folder reviewed during the inspection. I have attached a copy of my disposal summary sheet, with each item in 2002 highlighted and numbered 1-5, and I have attached the documentation for each item. 28 A signed copy of manifest #22091585 could not be found during the inspection. Signed copy of Manifest requested and received from TSDE Copy attached. 29 An exception report must be sent to the DTSC within 45 days if a signed manifest copy is not received. Copy of manifest and exception letter sent to DTSC attached. 43 This facility could not provide a contingency plan during the inspection. Our Emergency Action Manual, last revised 4-20-1996, has been updated to reflect recent changes in plant personnel and is available for review. I have attached a copy of the manual's contents page, the section on handling spills and the list of emergency contacts. A list of emergency contacts is also maintained in our security office, which is staffed round the clock. Security personnel are in contact with key hourly and management personnel via inter- plant 2-way radio and cell phone to ensure a timely response to any emergency. 49 Training records could not be provided during the inspection. Copies attached. 75 Universal waste lamps, batteries and aerosol cans were found without a Universal waste label. Provide proper labeling for all universal waste. Item has been corrected. #77 The universal waste found at the facility has been onsite an unknown amount of time. Properly dispose of the universal waste. See attached manifest #23374884 for disposal of aerosol cans and BOL for removal of batteries.
