Another LDR notification was found on site dated 6/28/16 for manifest#005328927SKS, but no <br /> corresponding consolidated manifest receipt was available. <br /> Hazardous waste generators shall retain copies of all manifests signed off by the disposal facility and all receipts <br /> used in a consolidated manifesting procedure on site for three years and have them readily available for review. <br /> Immediately locate a copy of all manifests and receipts for the last three years, maintain them on site, and submit <br /> copies to the EHD. <br /> This is a repeat violation, Class II. <br /> 301 CCR 66262.34(d)(2) Facility not maintained to minimize the release of a hazardous waste. <br /> -A large piece of metal was observed in the yard north of the Blasting Department covered in a thin layer of oil. <br /> According to Mr. Van Teslaar, the metal is a part of the press and remains uncovered in the yard, exposed to the <br /> elements, until it is needed. <br /> -Used oil was observed accumulated on the tops of several drums inside the hazardous waste storage area. Oily <br /> staining and oily contaminated absorbent was observed all over the ground in the hazardous waste storage area. <br /> -Used oil was observed leaking from the press machine near the Trim Department, along with an accumulation of oil <br /> saturated absorbent on the floor around the machine. <br /> -Oily drips, oily accumulation on the floor, and oil saturated absorbent was observed around machinery in various <br /> departments, including the Mill/Calender Department, Machine Shop, and Maintenance Shop. <br /> Facilities shall be maintained and operated to minimize the possibility of a fire, explosion, or release of hazardous <br /> waste to air, soil, or surface water which could threaten human health or the environment. Immediately clean all oil <br /> spills and stained areas, remove all contaminated soils and absorbents, and manage according to Title 22 hazardous <br /> waste regulations. Submit a statement and supporting documentation explaining how this waste was managed. <br /> This is a repeat violation, Class II. <br /> 403 CCR 66262.34(d)(2) Failed to keep hazardous waste containers closed except when adding or removing hazardous <br /> waste. <br /> -An open-top, black plastic pan of oily solids was observed in the yard south of the Blasting Department. <br /> -Used oil was observed accumulated on the tops of several drums inside the hazardous waste storage area. <br /> -A one gallon bucket of used oil was observed with no lid in the hazardous waste storage area. <br /> All hazardous waste containers shall be closed at all times except when adding or removing waste. Immediately close <br /> these containers and ensure all hazardous waste containers are closed when not adding or removing waste. <br /> This is a repeat violation, Class II. <br /> Page 7 of 10 <br />
