Several 2012 and 2013 hazardous waste manifests were found on site without a signed off <br /> copy from the destination facility. Hazardous waste generators shall retain copies of all manifests <br /> signed off by the destination facility on site for three years and have them readily available for review. If <br /> the generator did not receive a copy of the manifest with the handwritten signature of the owner or <br /> operator of the facility to which the generator's waste was submitted within 60 days of the date the waste <br /> was accepted by the initial transporter, the generator shall submit a legible copy of the missing manifest, <br /> with some indication that the generator has not received confirmation of delivery. This information shall <br /> be submitted to: <br /> DTSC Report Repository <br /> Generator Information Services Section <br /> P.O. Box 806 <br /> Sacramento, CA 95812-0806 <br /> Immediately locate a copy of the 2012 and 2013 manifests or prepare and submit the required <br /> information to DTSC. By September 7, 2013, submit to the EHD a copy of the signed off copy of the <br /> manifests or a copy of the information submitted to DTSC. <br /> 128. Bills of lading for the recycling of used oil filters for 2012 and 2013 were not found on site. A copy <br /> of each bill of lading must be kept on site for at least three years. Immediately locate a copy of all bills of <br /> lading for used oil filters for 2012 and 2013, maintain them on site, and submit copies to the EHD by <br /> September 7, 2013. <br /> 301. Derric Juano stated that the aqueous portion of the three stage separator is discharged to the <br /> stormwater system when rainwater fills the separator. The separator is part of a closed loop power <br /> steam washer system used to clean oily equipment and parts. Facilities shall be maintained and <br /> operated to minimize the possibility of a fire, explosion, or release of hazardous waste to air, soil, or <br /> surface water which could threaten human health or the environment. Immediately stop discharging any <br /> contents of the three stage separator into the stormwater system, and manage according to Title 22 <br /> hazardous waste regulations.
