(#21 & #22) At the end of the month, please enter <br />the total number of tires/batteries that you have to the left of the Week 1 line. <br />Wash Rack/Sump Weekly Monitoring <br />From left to right (Weekly), please enter the estimated amount of sludge (sediment) <br />at the bottom of the wash rack. Do the same for the second stage that is the chamber <br />next to the rack and usually covered. Do the same for the third stage, if applicable. <br />The comments should be used for any informative remarks or observations. Finally, <br />the employee that did the weekly inspection will enter their initials. <br />List of Hazardous Waste at Maintenance Facilities <br />This area is extra space for entering any pertinent information to your inspection <br />(i.e., Note of corrections for any deficiencies; comments such as your yard needs new <br />drums; or additional space for more information from above that you can list by item <br />number; etc.). <br />Name of Supervisor/Phone The employee that completes the inspection shall sign <br />the form and enter the phone number of the station. <br />July 2018 <br />
