Manifest numbers: 005521089SKS (8/12/2016), 005028397SKS (9/14/2016), 005622289SKS (12/6/2016) 005523255SKS (12/19/2016), 005946972SKS (5/5/2017), 006099557SKS (5/23/2017), 006100846SKS (6/29/2017), 00619522SKS (8/22/2017), 006325813SKS (1/17/2018) and 006595006SKS (4/2/2018) were found without a signed off copy from the destination facility. Manifest numbers: 005106062SKS (11/9/2015), 005142895SKS (11/9/2015), 005328719SKS (6/3/2016) and 005523396SKS (5/11/2017) were not found on site. Hazardous waste generators shall retain copies of all manifests for three years or until a copy signed off by the destination facility is received. The signed off copy shall be retained and made readily available for review for three years. If the generator did not receive a copy of the manifest with the handwritten signature of the owner or operator of the facility to which the generator's waste was submitted within 35 days of the date the waste was accepted by the initial transporter, the generator shall contact the transporter and/or the owner or operator of the designated facility to determine the status of the waste. If the generator did not receive a copy of the signed off copy of the manifest within 45 days of the date that the waste was accepted by the initial transporter, the generator shall submit an Exception Report that includes a legible copy of the missing manifest, and a cover letter signed by the generator explaining the efforts to locate the hazardous waste and the results of those efforts. Exception reports shall be submitted to: DTSC Report Repository Generator Information Services Section P.O. Box 806 Sacramento, CA 95812-0806 Immediately locate a copy of the missing manifests or prepare and submit an exception report to DTSC. Submit to the EHD a copy of the signed off copy of the manifests or a copy of the exception report submitted to DTSC. Copies of uniform manifests for the disposal of "ash" waste and metal fines were not found on site for 2017 and 2018. In addition, receipts indicate that on 12/12/2016, used oil was picked up and on 10/20/2016, used antifreeze and used oil were picked up. Records also show that used oil was picked up during 12/2017. These records were not found on site. Hazardous waste generators shall retain copies of all manifests signed off by the disposal facility and all receipts used in a consolidated manifesting procedure on site for three years and have them readily available for review. Immediately locate a copy of all manifests and receipts for the last three years and maintain them on site. Submit copies of the disposal records for the "ash" waste and metal fines waste for 2017 and 2018 to the EHD. Submit copies of disposal records for the used antifreeze and used oil waste for the missing dates listed above for 2016 and 2017 to the EHD. This is a repeat violation, Class II.
