The contingency plan for this facility did not include descriptions of emergency response to fire or explosion, a list of emergency equipment at the facility, or the signals used for the evacuation plan. A contingency plan shall include all of the following: -actions facility personnel shall take in response to fires, explosions, and unplanned release to air, soil, or surface water -arrangements agreed to by local police departments, fire departments, hospitals, contractors, and state & local emergency response teams -names, addresses & phone numbers (office & home) of the Emergency Coordinator -list of all emergency equipment at the facility (fire, spill, communications/alarms, decon), including location, physical description, and a brief outline of its capabilities -evacuation plan, including routes and signals to be used -telephone number of Cal EMA This information must be kept current at all times. Immediately update the contingency plan for this facility to include all of the necessary information and submit a copy of the updated plan to the EHD by August 1, 2011. 64. Copies of spent lead acid battery disposal records were not found on site. The generator shall retain on site for at least three years a legible copy of each manifest or bill of lading which identifies spent lead acid batteries shipped to a person who stores, uses, reuses, recycles, or reclaims the batteries or their components. Immediately locate a copy of all disposal records for spent lead acid batteries for the last three years, maintain them on site, and submit copies to the EHD by August 1, 2011. 85. Copies of universal waste disposal records were not found on site. A generator of universal waste (alkaline batteries, mercury containing equipment, fluorescent bulbs, electronic devices, CRTs, aerosol cans) shall keep a record of each shipment of universal waste on site for at least three years. The record may take the form of a log, invoice, manifest, bill of lading, or other shipping document, and shall include the following: - Name & address of destination facility - Quantity & type of universal waste - Date of departure/shipment Begin this practice immediately. ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE ($122).
