(continued) <br />leak at the earliest opportunity. Immediately remove and properly dispose of all hazardous waste from <br />spill containment, and maintain all spill containment clean and dry at all times. <br />45. The contingency plan for this facility did not include descriptions of emergency response to fire or <br />explosion, a list of emergency equipment at the facility, or the signals used for the evacuation plan. A <br />contingency plan shall include all of the following: <br />-actions facility personnel shall take in response to fires, explosions, and unplanned release to air, soil, <br />or surface water <br />-arrangements agreed to by local police departments, fire departments, hospitals, contractors, and state <br />& local emergency response teams <br />-names, addresses & phone numbers (office & home) of the Emergency Coordinator <br />-list of all emergency equipment at the facility (fire, spill, communications/alarms, decon), including <br />location, physical description, and a brief outline of its capabilities <br />-evacuation plan, including routes and signals to be used <br />-telephone number of Cal EMA <br />This information must be kept current at all times. Immediately update the contingency plan for this <br />facility to include all of the necessary information and submit a copy of the updated plan to the EHD by <br />August 1, 2011. <br />64. Copies of spent lead acid battery disposal records were not found on site. The generator shall retain <br />on site for at least three years a legible copy of each manifest or bill of lading which identifies spent lead <br />acid batteries shipped to a person who stores, uses, reuses, recycles, or reclaims the batteries or their <br />components. Immediately locate a copy of all disposal records for spent lead acid batteries for the last <br />three years, maintain them on site, and submit copies to the EHD by August 1, 2011. <br />85. Copies of universal waste disposal records were not found on site. A generator of universal waste <br />(alkaline batteries, mercury containing equipment, fluorescent bulbs, electronic devices, CRTs, aerosol <br />cans) shall keep a record of each shipment of universal waste on site for at least three years. The <br />record may take the form of a log, invoice, manifest, bill of lading, or other shipping document, and shall <br />include the following: <br />- Name & address of destination facility <br />- Quantity & type of universal waste <br />- Date of departure/shipment <br />Begin this practice immediately. <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE ($122). <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT END'S CURRENT HOURLY RATE. <br />EHD Inspector: <br />Received By: <br />Title: <br />CONTINUATION FORM <br />EHD 23-02-003(:;708/10/10 / <br />
