SELF MONITORING SAMPLE PROGRAM McCORMICK AND BAXTER COMPANY Page 2 SELF MONITORING SAMPLE PROGRAM CONDITIONS 1. Permittee shall send reports on a monthly basis. The flow meter shall be read daily, and the daily f low included in the monthly report. Reports shall be submitted to the Environmental Control Section no later than the tenth day of the following month for which monitoring was conducted. 2 . Reports shall indicate time, date and place of sampling, methods of analysis, and shall certify that such sampling and analysis is representative of normal operating cycles and expected pollutants discharged to the sewer. 3. Any, and all, additional groundwater sampling analytical data collected from the McCormick and Baxter site shall be submitted to the City of Stockton Environmental Control Section as part of the monthly report. 4. Permittee shall perform quality assurance program as specified in the City of Stockton"s, "Quality Assurance Manual for Self Monitored Industries" in conjunction with permittee-'s self monitoring sample program. 5. Reports shall contain the following certification statement signed by an authorized representative: "I certify under penalty of law that this document and all attachments were prepared under my direction or supervision in accordance with a system designed to assure that qualified personnel properly gather and evaluate the information submitted. Based on my inquiry of the person or persons who manage the system, or those persons directly responsible for gathering the information, the information submitted is, to the best of my knowledge and belief, true, accurate, and complete. I am aware that there are significant penalties for submitting false information, including the possibility of fine and imprisonment for knowing violations. "
