I had previously given this to Bill Little also. Upon reviewing the <br />assessment that you provided and comparing it to the model, I found these items that are missing (this may not <br />be a complete list): <br />1. Dates of inspections <br />2. Statement <br />I certify under penalty of law that this document and all attachments were prepared under my direction or <br />supervision in accordance with a system designed to assure that qualified personnel properly gather and evaluate <br />the information submitted. Based on my inquiry of the person or persons who mange the system, or those <br />persons directly responsible for gathering the information, the information submitted is, to be the best of my <br />knowledge and belief, true, accurate, and complete. I am aware that there are significant penalties for submitting <br />false information, including the possibility of fine and imprisonment for knowing violations. <br />3. Age of tank <br />4. Remaining service life <br />5. Method used to determine thickness <br />6. Corrosion protection equipment <br />7. Piping material, pumps, location, other ancillary equipment, lining of tank if applicable. <br />8. Material Storage: Material and characteristics: 191(g)(7)/192(k)(8) Specific Gravity: <br />9. Chemical capatiblity findings <br />10. Visual inspection of the interior of the tank <br />11. Tank parameters at the time of the inspection <br />12. Complete drawings of the tank <br />13. Secondary Containment assessment <br />14. Seismic Load Calculation <br />15.Tank integrity test <br />Please submit the revised assessment that completely addresses all of the sections noted on the model.
