Manifest 3329881 FLE was found without a signed off copy from the destination facility. -- Hazardous waste generators shall retain copies of all manifests signed off by the destination facility on site for three years and have them readily available for review. If the generator did not receive a copy of the manifest with the handwritten signature of the owner or operator of the facility to which the generator's waste was submitted within 35 days of the date the waste was accepted by the initial transporter, the generator shall contact the transporter and/or the owner or operator of the designated facility to determine the status of the waste. If the generator did not receive a copy of the signed off copy of the manifest within 45 days of the date that the waste was accepted by the initial transporter, the generator shall submit an Exception Report that includes a legible copy of the missing manifest, and a cover letter signed by the generator explaining the efforts to locate the hazardous waste and the results of those efforts. Exception reports shall be submitted to: DTSC Report Repository Generator Information Services Section P.O. Box 806 Sacramento, CA 95812-0806 " Immediately locate a copy of the missing signed manifest or prepare and submit an exception report to DTSC. Submit a copy of the exception report to this office. 79,83,84. Unable to determine the length of storage of two Universal Waste (UW) fluorescent lamps on upstairs storage and one UM -lamp behind light fixture in the one of the maintenance trenches in the maintenance shop. The UW -lamps were not stored in a container. -- UW -lamps shall be stored in a closed container that prevents breakage and release to the environment, and the container shall be labeled to identify the type of UW and the initial date of accumulation. UW -lamps shall be managed within one year of storage. * Immediately dispose of the above UW -lamps and submit a copy of the disposal receipt to EHD. A pamphlet for the Small HW Disposal Facility has been supplied to the generator during this inspection. NOTES (The following are not violations) The facility generated approximately 36.39 tons of HW in 2009. The inspection was conducted on October 14, 2010, but only a checklist was provided to the generator due to time constrain. This is the final version of the HW inspection report. Complete and submit a copy of the Return to Compliance Certification form (provided with this report) with a statement of how each violation was corrected, including supporting documentation by November 21, 2010 ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE ($122.
