Fremont, Stockton <br />SUMMARY OF VIOLATIONS <br />CLASS 1, CLASS 11, or MINOR -Notice to Comply) <br />49. (Repeat) Failed to conduct personnel training of HW spill prevention and response in accordance to <br />the contingency plan since 2007. <br />-- Facility shall provide in -class training or on-the-job training of new employee within six months of <br />their employment, annual review of the initial personnel training, maintain document and records of <br />training. <br />* Immediately provide personnel training per this section. Submit document verifying training to EHD <br />by November 21, 2010. <br />The following are Minor Violations - Notice to Comply: #11,14,60,61,62,79,83,84: <br />11. Observed absorbents on the floor of the maintenance shop, and stains on the floor below the <br />storage shelves at the maintenance shop. <br />-- Facilities shall be maintained and operated to minimize the possibility of a fire, explosion, or <br />release of hazardous waste to air, soil, or surface water which could threaten human health or the <br />environment. <br />* Immediately clean stains and manage according to Title 22 hazardous waste regulations. <br />Submit to EHD by November 21, 2010 a statement and supporting documentation explaining how this <br />waste was managed, and any corrective measures taken to minimize the release of HW in the future. <br />14. Observed eye -wash station at fiberglass shop not in working condition. <br />-- Facility shall be equipped with portable fire extinguishers, fire control equipment, spill control <br />equipment, and decontamination equipment in working condition in response to HW release. <br />* Immediately provide working eye -wash station at the fiberglass shop, and maintain all the <br />above-mentioned equipment in working condition per this section.
