According to the training <br /> records on site, Jeff Hughes, Mike Fouts, and Rodney Dirks did not receive the mandatory "Hazardous Waste- EMS- <br /> Spill Response"training, which was due in April 2014. Personnel who handle hazardous waste shall be trained within <br /> six months of assignment and take part in an annual review of the initial training received. At a minimum, the training <br /> program shall be designed to ensure that facility personnel are able to respond effectively to emergencies by <br /> familiarizing them with emergency procedures, emergency equipment, and emergency systems, including where <br /> applicable: <br /> 1. procedures for using, inspecting, repairing, and replacing facility emergency and monitoring equipment; <br /> 2. key parameters for automatic waste feed cut-off systems; <br /> 3. communications or alarm systems; <br /> 4. response to fires or explosions; <br /> 5. response to ground-water contamination incidents; and <br /> 6. shutdown of operations. <br /> The owner or operator shall maintain the following documents and records at the facility: <br /> 1. the job title for each position at the facility related to hazardous waste management, and the name of each <br /> employee filling each job; <br /> 2. written job description for each position listed above; <br /> 3. written description of the type and amount of both introductory and continuing training that will be given to each <br /> person filing positions listed above; <br /> 4. records that document that the training has been given to and completed by facility personnel. <br /> Training records on current employees shall be kept until closure of the facility. Training records on former employees <br /> shall be kept for at least three years from the date the employee last worked at the facility. Immediately provide <br /> employees with initial or refresher hazardous waste training. Submit a copy of the updated records to the EHD. This is <br /> a repeat violation, Class II. <br /> 113 Failed to keep signed copy of manifests from the designated facility for three years. Manifest number 012413331JJK <br /> (4/15/14)was found without a signed off copy from the destination facility. Hazardous waste generators shall retain <br /> copies of all manifests signed off by the destination facility on site for three years and have them readily available for <br /> review. If the generator did not receive a copy of the manifest with the handwritten signature of the owner or operator of <br /> the facility to which the generator's waste was submitted within 35 days of the date the waste was accepted by the initial <br /> transporter, the generator shall contact the transporter and/or the owner or operator of the designated facility to <br /> determine the status of the waste. If the generator did not receive a copy of the signed off copy of the manifest within 45 <br /> days of the date that the waste was accepted by the initial transporter, the generator shall submit an Exception Report <br /> that includes a legible copy of the missing manifest, and a cover letter signed by the generator explaining the efforts to <br /> locate the hazardous waste and the results of those efforts. Exception reports shall be submitted to: <br /> DTSC Report Repository <br /> Generator Information Services Section <br /> P.O. Box 806 <br /> Sacramento, CA 95812-0806 <br /> Immediately locate a copy of the missing manifest or prepare and submit an exception report to DTSC. By October 11, <br /> 2014, submit to the EHD a copy of the signed off copy of the manifest or a copy of the exception report submitted to <br /> DTSC.
