Manifest number 012413331JJK <br /> (4/15/14)was found without a signed off copy from the destination facility. Hazardous waste generators shall retain <br /> copies of all manifests signed off by the destination facility on site for three years and have them readily available for <br /> review. If the generator did not receive a copy of the manifest with the handwritten signature of the owner or operator of <br /> the facility to which the generator's waste was submitted within 35 days of the date the waste was accepted by the initial <br /> transporter, the generator shall contact the transporter and/or the owner or operator of the designated facility to <br /> determine the status of the waste. If the generator did not receive a copy of the signed off copy of the manifest within 45 <br /> days of the date that the waste was accepted by the initial transporter, the generator shall submit an Exception Report <br /> that includes a legible copy of the missing manifest, and a cover letter signed by the generator explaining the efforts to <br /> locate the hazardous waste and the results of those efforts. Exception reports shall be submitted to: <br /> DTSC Report Repository <br /> Generator Information Services Section <br /> P.O. Box 806 <br /> Sacramento, CA 95812-0806 <br /> Immediately locate a copy of the missing manifest or prepare and submit an exception report to DTSC. Submit to the <br /> EHD a copy of the signed off copy of the manifest or a copy of the exception report submitted to DTSC. This is a repeat <br /> violation, Class 11. <br /> 116 Failed to follow all consolidated manifesting procedures. Copies of hazardous waste disposal records for parts washer <br /> waste were not found on site. Hazardous waste generators shall retain copies of all manifests signed off by the disposal <br /> facility and all receipts used in a consolidated manifesting procedure on site for three years and have them readily <br /> available for review. Immediately locate a copy of all manifests and receipts for the last three years, maintain them on <br /> site, and submit copies to the EHD. This is a repeat violation, Class 11. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by October 11, 2014. <br /> Consent to perform the inspection, take photos and collect samples was given by Steve Schamaun, Project <br /> Engineer. <br /> This facility generated 31.88 tons of waste in the last calendar year (Jan-Dec 2013). The facility has so far <br /> generated 20.26 tons of hazardous waste in 2014. <br /> Please be aware as of January 1, 2013, all businesses are required to submit all hazardous materials information <br /> online to the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov.
