(209) 468-3433 Web: www.siaov.org/ehd/unitiii.htmi <br />CONTINUATION FORM <br />Page: 3 of 6 <br />OFFICIAL INSPECTION REPORT <br />Date: 06/07/11 <br />Program: HW <br />Facility Address: 120 Frank West Cir., Stockton <br />SUMMARY OF VIOLATIONS <br />CLASS I, CLASS II, or MINOR -Notice to Comply) <br />ROUTINE HAZARDOUS WASTE GENERATOR INSPECTION <br />"All photos were taken by Mike Fouts at the request of Mr. Takemori. According to Mr. Takemori, <br />copies will be provided to the EHD after Teichert administrative approval. <br />Class II violations: (18) <br />18. The used antifreeze tank was observed behind the shop with an accumulation start date of January <br />20, 2011. Facilities who generate 1000 kg or more of hazardous waste per month and or exceed 6000 <br />kg of waste stored on site at any time may not store waste on site for longer than 90 days. Immediately <br />contact a licensed hazardous waste hauler to dispose of this waste under manifest and submit a copy of <br />the manifest to the EHD by July 7, 2011. <br />REPEAT VIOLATION <br />Notice to Comply - Minor violations: (7, 32, 33, 35, 45, 49, 85) <br />7. Metal fines generated from metal grinding, cutting, and drilling are being recycled as scrap metal <br />without first determining particle size or hazardous properties. Oily rags were found in the trash can. <br />The oily absorbent on site is being sent to Altamont Landfill as designated waste. Any person who <br />generates a waste shall determine if the waste is a hazardous waste. Immediately stop recycling metal <br />fines, make a hazardous waste determination for the metal fines and oily waste, and manage them <br />according the Title 22 hazardous waste regulations. Metal particles 100 microns or smaller must be <br />handled as hazardous waste if the metal is determined to be a hazardous waste. Submit a statement <br />and supporting documentation explaining how these wastes were managed by July 7, 2011. <br />32. A tank assessment was not available on site for the used antifreeze tank. A hazardous waste <br />generator shall obtain and keep on site a written assessment for each hazardous waste tank at the <br />facility. This assessment shall be reviewed and certified by an independent, qualified, professional <br />engineer, registered in California, and shall attest to the tank system's integrity. The assessment shall <br />determine that the tank system is adequately designed and has sufficient structural strength and <br />compatibility with the waste stored, and shall be written in accordance with Title 22 regulations. <br />Immediately obtain a written tank assessment for the used antifreeze tank at this facility in accordance <br />with Title 22 regulations, maintain a copy on site at the facility, and submit a copy of the certified <br />assessment to the EHD. <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE ($122). <br />THIS FACILITY IS JECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br />EHD Ins or: <br />Received By- <br />Title: <br />?I <br />EHD 23-02-0 v 08/10/10 CONTINUATION FORM <br />
