V CVSHealth
Ms. Florido
November 2, 2018
Page 5
State Regulated Wastes; and Universal Waste. The program prohibits any free liquids, whether
contained in a self-sealing bag or not. Any item that is leaking or likely to leak must be double-
bagged with absorbent to ensure that no free liquids are placed into the hazardous waste
container
It appears that during your inspection, you observed a hazardous waste tote with a label
which did not include check marks indicating all the contents, and processed items which had
leaked, but had not been double-bagged with absorbent added. CVS's hazardous waste
management program requires stores to process damages daily. Each item must be placed into
self-sealing hazardous waste bags that serve as the initial container for each waste item, and they
are required to remain closed except when adding waste. The program prohibits any free liquids,
whether contained in a self-sealing bag or not. Any item that is leaking or likely to leak must be
double-bagged with absorbent to ensure that no free liquids are placed into the hazardous waste
container. Accordingly, appropriate store colleagues have been retrained on the proper
unsaleables/damages process, including the importance of placing bagged hazardous waste items
into the correct hazardous waste tote in a timely manner, processing damages daily, and using
only hazardous waste totes to store processed hazardous waste items. We have confirmed that
the UW that doesn't fit into the hazardous waste tote, was labeled and placed in the hazardous
waste area.
In regard to the open blue bin(blue akrobin) labeled"Weekly StrongPak Accumulation
Bin"referenced in your inspection report, holding pharmaceuticals. This tray is a temporary
storage container used to hold pharmaceutical items awaiting evaluation. The tray is not labeled
with a hazardous waste label because the items contained therein are product and have not yet
been processed to remove from inventory nor evaluated to determine whether they will be
managed through CVS's reverse distribution vendor or will be managed as waste at the store
level. Items temporarily stored in this tray are evaluated on a weekly basis by pharmacy
personnel. Upon evaluation, they are either packaged for transport to CVS's reverse distribution
vendor or are managed though the store's Hazardous Waste Management Program. We have
confirmed that the pharmacy hazardous waste tote label is currently listed the contents.
Annual Training/Training Records
Regarding the store's hazardous waste training, CVS has developed the attached matrix
which lists job duties related to hazardous waste management, a description of such duties, and a
list of store-level responsible employees by category and job code. This matrix supplements the
Training Program Plan and Course Overview on pages 6-7 of the Program Overview(attached)
and applies to all retail store locations. The type and amount of introductory and continuing
training provided to store employees is discussed in the Program Overview.
Item#119 relates to maintain training records onsite. CVS requires all store colleagues to
be trained annually to CVS's hazardous waste management program through one or more
Cn pharmacy / caremark /minute clinic/ specialty
