Section 66265.16, Division 4.5, Title 22, California Code of Regulations: <br />Personnel training in hazardous waste management procedures was inadequate. <br />8. Section 6626534 (a), Division 4.5, Title 22, California Code of Regulations: Inside <br />the mixing room, personnel involved in pouring hazardous waste into drums do <br />not have immediate access to an internal alarm or communication device. <br />H. SCHEDULE OF CO ONS: <br />Violations Effective immediately, all hazardous waste containers shall be <br />#1, #2, #3: labeled with the following information: <br />a) The start date of accumulation. <br />b) The words "HAZARDOUS WASTE". <br />c) Composition and physical state of waste. <br />d) Statement(s) which will call attention to the particular <br />hazardous properties of the waste. <br />e) Name and address of person producing waste. <br />Violation #4: Effective immediately, a copy of each biennial report on site <br />according to referenced code section. <br />Violations Effective immediately, the contingency plan shall be completed <br />#5 and #6: according to referenced code section. <br />Violation #7: Within 30 days of receipt of this report, complete and maintain <br />employee training records according to referenced code section. <br />Violation #8: Within 30 days of receipt of this report, access to communications or <br />alarm systems shall be available to personnel according to <br />referenced code section. <br />When all violations have been corrected, please send written certification to this office at <br />the letterhead address. This certification shall contain the sample certification statement <br />indicated on Attachment L <br />PHS-EHD may conduct a follow-up inspection at the facility to verify compliance. <br />ig <br />
