(66265.190(b)) <br />r <br />Comment <br />If the answer is yes, for all Hw tanks, to either of the two preceding questions, then <br />skip pages 8 through 16, since they are not applicable. <br />Are HW or treatment reagents placed in tanks so <br />they do not cause the tank, ancillary equipment or <br />the secondary containment system to rupture, <br />corrode, leak, or otherwise fail? [66265.194(a))_ <br />Are controls and practices used to prevent spills <br />and overflows, including: [66265.194(b)-] <br />(1) Spill prevention controls (e.g., check <br />valves and dry discount couplings)? <br />(2) Overfill prevention controls (e.g., level <br />sensing devices, high level alarms, automatic <br />feed cutoff, or bypass to a standby tank)? <br />(3) Sufficient freeboard in uncovered tanks to <br />prevent overtopping by wave or wind action or <br />by precipitation? <br />If a leak or spill has occurred, was 66265.196 <br />cOmPlied with (see page 9)? [66265.194(c) <br />Does transfer or storage of HW in tanks comply <br />with 66265.17(b)? [66265.194(d)] <br />Are daily inspections done for the following: <br />(66265.195(a)-) <br />(1) Discharge control equipment (e.g., feed <br />cutoff, bypass, and drainage systems)? <br />(2) Corrosion or releases of waste in the above <br />ground portions? <br />(3) Data gathered from monitoring and leak <br />detection equipment (e.g., pressure and <br />temperature gauges, monitoring wells)? <br />(4) Construction materials and area surrounding <br />tank and secondary containment for erosion or <br />signs of releases (e.g., wet spots, dead <br />vegetation) ? <br />(4) For uncovered tanks, the level of waste to <br />comply with 66265.194(b)(3)? <br />Are cathodic protection systems inspected six <br />months after initial installation and then <br />annually?
