Date June 29 1995 <br />W D lop 0 <br />IM <br />This monitoring program must be kept at the UST location at all times. The information on this monitoring <br />program are conditions of the operating permit. The permit holder must notify San Joaquin County <br />Environmental Health Division, phone (209) 468-3420 within 30 days of any changes to the monitoring <br />procedures, unless required to obtain approval before making the change. <br />Required by Sections 2632(d) and 2641(h) CCR. <br />Facility Name Country Club Shell <br />Facility Address 2575 Country Club, Stockton <br />If an unauthorized release occurs, how will the hazardous substance be cleaned up? <br />Note: If released hazardous substances reach the environment, increase the fire or <br />explosion hazard, are not cleaned up from the secondary containment within 8 <br />hours, or deteriorate the secondary containment, then San Joaquin County <br />Environmental Health Division, phone (209) 468-3420 must be notified within 24 <br />hours. For small spills the on site personnel will use absorbent material to clean up the <br />released material. In the event of a larger spill, the dealer will call 9-1-1 (if necessarX� <br />his/her Shell Representative and the Shell Engineer to assist in the emergency. <br />2. Describe the proposed methods and equipment to be used for removing and properly <br />disposing of any hazardous substances. <br />Spent absorbent will be placed into an approved container and disposed of in accordance <br />with all Local State. and Federal laws and regulations. Any additional equipment will <br />be provided be the responding contractor. <br />3. Describe the location and availability of the required cleanup equipment in item 2 above. <br />Absorbent is maintained on site and restocked as needed. Any additional equipment is <br />maintained by the contractor, and available on an as needed basis. <br />4. Describe the maintenance schedule for the cleanup equipment. <br />Absorbent is inspected weekly and reordered as needed. <br />5. List the name(s) and title(s) of the person(s) responsible for authorizing any work <br />necessary under the response plan: <br />Steve Griffen / Station Manager <br />Jeff Granberry / Shell Project Engineer <br />Jeff Byrum / Shell District Engineer <br />Dan Kirk / Shell Environmental Engineer <br />
