CASE SUMMARY <br />DIRECTOR <br />Donna Heran, REHS <br />ASSISTANT DIRECTOR <br />Laurie Cotulla, REHS <br />PROGRAM COORDINATORS <br />Mike Huggins, REHS, RDI <br />Margaret Lagorio, REHS <br />Robert McClellon, REHS <br />Jeff Carruesco, REHS, RDI <br />Kasey Foley, REHS <br />On December 6, 2007, Mr. Garrett Backus, Senior Registered Environmental Health <br />Specialist (RENS), San Joaquin County Environmental Health Department (EHD), <br />performed a routine underground storage tank (UST) inspection (Attachment 1) and <br />witnessed the annual monitoring system certification, leak detector testing, and spill <br />containment testing. The diesel fill sump annular sensor failed and was replaced and <br />retested by the service technician. Mr. Backus found that the monitoring plan was <br />incomplete. A monitoring system certification test report was submitted on January 16, <br />2008, after 30 days (Attachment 2). <br />On December 7, 2007, a revised scope of work was submitted for an existing permit by <br />BZ Service Station Maintenance for the replacement of the diesel fill sump annular <br />sensor (Attachment 3). <br />On January 6, 2009, Mr. Backus performed a routine UST inspection (Attachment 4) but <br />not the annual monitoring system certification, leak detector testing, and spill container <br />testing because they had not yet been scheduled (although they were already one <br />month past due). A current copy of the financial responsibility documents and <br />monitoring plan were not on file with EHD. Nine alarms from the last 12 months were <br />found without maintenance records or any explanation as to how the alarms were <br />corrected. The designated operator monthly inspections from May through December <br />2008 were performed by Mr. Ian Hall, who was not listed on the facility's designated <br />operator statement. The July, October, November, and December 2008 designated <br />operator monthly inspection reports did not have complete alarm histories attached, and <br />one employee did not have current training by the designated operator. <br />On April 22, 2009, Mr. Backus sent a follow up letter requesting the items noted on the <br />January 9, 2009, routine UST inspection report (Attachment 5).
