complete and submit a Xerxes Underground Chemical Tank Application for Approval and <br />that Xerxes grants written approval of that application. The approved application must be <br />attached to the Underground Chemical Tanks Limited Warranty as Exhibit A. A copy of the <br />written approval from Xerxes must be submitted to this office. <br />LM W AW procedure describing proposed actions the facility plans to implement after fter each <br />r%vk . 'M "specific decontamination event" in order to prevent cross contamination between events. <br />4. Qualified installers must possess or work under the direct and personal supervision of an <br />individual physically present at the work site, at all times while work is being performed, <br />i� who possesses all of the following: <br />a. One of several permissible valid, current contractor's licenses issued by the <br />Contractors State License Board (may be issued to the installation company), and <br />b. All valid, current California UST certificates issued by the International Code <br />Council (ICC), that are applicable to the work being performed, and <br />c. A valid, current certificate of training from the manufacturer(s) of the <br />component(s) being tested, repaired, or serviced. <br />And in addition <br />d. Each person installing or testing any component must hold a certificate of training <br />�p the manufacturer of that component. <br />Copies of all above mentioned certificates and licenses must be submitted to this office for <br />review. All deficiencies must be addressed before this scope of work can be approved. Provide <br />all necessary and requested supporting documentation to this office for review. Once the <br />information has been received, you may be contacted if further information is needed.
