Monitoring system certification test results (12-21-10, 1-21-11) and secondary containment test <br />results (5-9-12) were not found on site. These records shall be kept on site for at least 3 years. <br />Immediately locate and maintain these documents on site. <br />202. A copy of the underground storage tank monitoring plan was not found on site. The monitoring <br />plan shall be retained on site at all times. Immediately locate and retain a copy on site. <br />205. The March 2012 through July 2012 and November 2012 through January 2013 designated <br />operator monthly inspection reports were not found on site. Designated operator monthly inspection <br />reports for the previous twelve months shall be retained on site. Submit copies to the EHD by April 8, <br />2013 and maintain a copy on site. <br />206. The designated operator employee training records were not available on site. The designated <br />operator shall train facility employees for which he or she is responsible in the proper operation and <br />maintenance of the UST system once every 12 months. The training shall include, but is not limited to: <br />1. Operation of the UST system in a manner consistent with the facility's best management <br />practices <br />2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring <br />plan <br />3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br />4. Name of the contact person(s) for emergencies and monitoring equipment alarms <br />Ensure that employees have been trained by the designated operator, maintain the list on site, and <br />submit a copy of the training records to the EHD by April 8, 2013. <br />311. The 91 octane product line leak detector failed to detect a leak when tested. All line leak <br />detectors shall be capable of detecting a 3 -gallon per hour leak at 10 psi. Immediately have a properly <br />licensed, trained, and certified contractor repair or replace the failed leak detector (LG 113-x listed, if <br />applicable) and obtain a permit within one business day from the EHD. If the failed leak detector can't <br />be replaced immediately, there is a possibility that the 91 UST system may be red tagged to prevent fuel <br />inputs. <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br />EHD Inspector: <br />Garrett Backus (209) 468-2986 <br />Received By: <br />),-C-( tel lli-, <br />Title: <br />EHD 23-02-003 Rev 04/19/12 CONTINUATION FORM <br />
