The following is a Minor- Notice to Comply: <br /> #203, and the following are Class II Repeat violations: 104, 106,202, 302/308, 311 <br /> 104. Financial responsibility documents have not been submitted to the EHD. Current financial <br /> responsibility documents are required to be submitted annually. A copy of the financial responsibility <br /> documents dated January 10, 2012 were received on site. <br /> 106. The response plan on site are not current and not approved by the EHD. The monitoring and <br /> response plans must be current and approved by the EHD. A current copy of the completed response <br /> plan was received on site by EHD, and has been approved by the EHD. <br /> 202. Approved copies of the monitoring plan was not found on site. Copies of these plans shall be <br /> retained on site at all times. This violation was corrected on site. <br /> 203. Approximately quarter cup of diesel in the middle diesel UDC pan was observed during this <br /> inspection and it was not recorded. Any release from the primary containment that does not escape the <br /> secondary containment and was cleaned up within eight hours of being detected shall be recorded with <br /> the following information: <br /> 1. Operators name and phone number <br /> 2. List of types, quantities, and concentrations of hazardous substances released <br /> 3. Description of actions taken to control and clean the release <br /> 4. Method and location of disposal of the released substances <br /> 5. Description of actions taken to repair and prevent future releases <br /> 6. Description of method used to reactivate interstitial monitoring after repair or replacement of <br /> primary containment <br /> Immediately clean up the above spill and properly dispose of the waste properly, and record this release <br /> with the information required, and maintain it on site with the facility's monitoring records. Submit a copy <br /> of the recordable release statement to the EHD by Apirl 9, 2012. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br /> THIS FACILITY IS SUB ECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> Inspector. eceiv By.
