Hazelton, Stockton Program: UST <br /> SUMMARY OF VIOLATIONS <br /> CLASS 1,CLASS 11,or MINOR-Notice to Comply) <br /> 302 & 308. Secondary containment test boots of the diesel line(s) between the middle and south diesel <br /> dispenser pans were not pulled back, and the shrader valve in the middle dispenser pan was found <br /> pointing up; flow gradient of the above diesel line is not known. Any potential release from the primary <br /> containment into the secondary containment in the piping does not flow into a collection sump or is not <br /> detected at the closest monitoring point. All secondary containment systems shall be sloped so that all <br /> releases will flow to a collection sump located at the low point of the underground piping. Immediately <br /> contact a properly licensed, trained, and certified contractor to ensure that any releases in the piping <br /> flows into the piping sump with the closest monitoring point. Submit evidence of correction to EHD by <br /> Apirl 9, 2012. <br /> 311. The line leak detector failed to detect a leak when tested. All line leak detectors shall be capable <br /> of detecting a 3-gallon per hour leak at 10 psi. The service technician installed a new siphon jet, <br /> retested it, and it passed. <br /> NOTES <br /> Explained to David Bylow and technician on site that snap taps should be removed after leak detector <br /> test. Technician left the snap taps installed in place at the conclusion of this inspection. <br /> I gave technician and also James Payton who was on site the opportunity to pull back the secondary <br /> containment test boots, but the correction was not made. <br /> Complete and submit a copy of the Return to Compliance Certification form with a statement of how <br /> each violation was corrected, including supporting documentation, by April 9, 2012. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CU RENT HOURLY RATE. <br /> EHD Inspector: ed By: Title: <br /> Jeffrey Wong (209) 468-0335 1 n"""V=:::::�A,P"' I <br /> EHD 23-02-003 Rev 07/12/11 CONTINUATION FORM <br />
