Facilty alarm log documentation and a maintenance record for the L12 Dispenser Pan 4 (pump 7/8) <br /> Fuel Alarm on March 5, 2012, were not found on site. These records shall be kept on site for at least 3 <br /> years. Immediately locate and maintain the facility alarm log documentation and maintenance records <br /> for the L12 Dispenser Pan 4 Fuel Alarm from March 5, 2012, on site and submit copies to the EHD by <br /> November 22, 2012. <br /> 203. Approximately one pint of diesel in the diesel turbine sump and approximately one cup of diesel in <br /> the dispenser pan 4 (pump7/8) on October 22, 2012 was not recorded. Any release from the primary <br /> containment that does not escape the secondary containment and was cleaned up within eight hours of <br /> being detected shall be recorded with the following information: <br /> 1. Operator's name and phone number <br /> 2. List of types, quantities, and concentrations of hazardous substances released <br /> 3. Description of actions taken to control and clean the release <br /> 4. Method and location of disposal of the released substances <br /> 5. Description of actions taken to repair and prevent future releases <br /> 6. Description of method used to reactivate interstitial monitoring after repair or replacement of <br /> primary containment <br /> Immediately record this release with the information required, and maintain it on site with the facility's <br /> monitoring records. Submit a copy of the recordable release statement to the EHD by November 22, <br /> 2012. <br /> 204. The designated operator failed to document all the alarms from the attached alarm history on the <br /> March 23, 2012 designated operator monthly inspection report and failed to check that they were <br /> responded to appropriately. The missing alarms include: L12 Dispenser Pan 4 (pump 7/8) fuel alarm <br /> from March 5, 2012. On the September 14, 2012, designated operator report, the designated operator <br /> documented that all sensor alarms from August 21, 2012, were done by Afford-a-test. ElitelV conducted <br /> the testing on August 21, 2012 under permit SR0065393.
