Ham Ln. Lodi, CA 95240 Program: UST <br /> SUMMARY OF VIOLATIONS <br /> CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> documented and responded to appropriately, and attach a copy of the alarm history with documentation <br /> taken in response to any alarms to the monthly report. Ensure that designated operators performing <br /> monthly inspections at this facility are correctly including all of the required information on the reports. <br /> 204. The designated operator failed to check and note the correct test dates for the monitoring system <br /> certification testing and spill container testing on the September and October 2012 designated operator <br /> monthly inspection reports. Monitoring system certification testing and spill container testing were listed <br /> as August 21, 2012, but should have been listed as October 3, 2011. The designated operator failed to <br /> check and note the correct test dates for the secondary containment testing on the November 11, 2011 <br /> through October 21, 2012 designated operator monthly inspection reports. Secondary containment <br /> testing was listed as September 1, 2011, but should have been listed as September 6, 2011. During the <br /> monthly inspection, the designated operator shall check that all required testing and maintenance for the <br /> UST system have been completed and document the dates they were done. Ensure that designated <br /> operators performing monthly inspections at this facility are including all of the required information on <br /> the reports. <br /> Note: The designated operator report dated January 24, 2011 should be dated January 24, 2012. The <br /> January 24, 2012 alarm history report was attached to the report dated January 24, 2011. Remind your <br /> designated operator to correctly fill out the date at the top of the monthly designated operator report. <br /> 306. A fuel leak was found at the diesel filter in dispenser pan 4 (pump 7/8). All primary containment <br /> must be product tight. Immediately clean up the spilled fuel and properly dispose of it per Title 22 <br /> hazardous waste regulations. Have a properly licensed, trained, and certified contractor fix the leak. If <br /> the leak is due to failure other than the tightening the diesel filter, the scope of work may require a permit <br /> and inspection of the EHD. If the failed component can't be repaired or replaced immediately, there is a <br /> possibility that the diesel UST system may be red tagged to prevent fuel inputs. <br /> 310. Approximately one quart of liquid was observed in the 87 turbine sump. If water could enter into the <br /> secondary containment by precipitation or infiltration, it must be removed and disposed of properly. <br /> Immediately remove this liquid, make a hazardous waste determination per Title 22 hazardous waste <br /> regulations, and manage it accordingly.
