CLASS II, or MINOR -Notice to Comply) <br />On site to witness the annual monitoring system certification testing and conduct the routine inspection. <br />117. The L2 91 turbine sump veeder root 794380-208 sensor wire was replaced on 1-23-12 under Elite <br />IV work order #26004 and a permit was not obtained from the EHD. Emergency repairs relating to UST <br />system monitoring and release detection may be performed by a properly licensed, trained, and certified <br />contractor pursuant to the EHD's policy and a permit obtained within one business day from the EHD. All <br />other repairs must have a permit submitted to and approved by the EHD prior to beginning work. <br />Immediately obtain a permit from the EHD for the replaced L2 91 turbine sump veeder root 794380-208 <br />sensor wire. A "Failure to Obtain a Permit" penalty applies. <br />201. The maintenance record for the L3 diesel turbine sump fuel alarm from 1-23-12 was not found on <br />site. Maintenance records shall be kept on site for at least 3 years. Immediately locate and maintain the <br />maintenance record for the L3 diesel turbine sump fuel alarm from 1-23-12 on site and submit copies to <br />the EHD by 7-6-12. <br />203. Approximately four gallons of diesel was found in the #5/6 under dispenser containment sump on <br />6-6-12 was not recorded. Any release from the primary containment that does not escape the secondary <br />containment and was cleaned up within eight hours of being detected shall be recorded with the <br />following information: <br />1. Operator's name and phone number <br />2. List of types, quantities, and concentrations of hazardous substances released <br />3. Description of actions taken to control and clean the release <br />4. Method and location of disposal of the released substances <br />5. Description of actions taken to repair and prevent future releases <br />6. Description of method used to reactivate interstitial monitoring after repair or replacement of <br />primary containment <br />Immediately record this release with the information required, and maintain it on site with the facility's <br />monitoring records. Submit a copy of the recordable release statement to the EHD by 7-6-12. <br />204.
