A copy of the 12-10-10 alarm history report was instead <br />attached to the 4-20-12 designated operator monthly inspection report. During the monthly inspection, <br />the designated operator shall review the alarm history for the previous month, check that each alarm <br />was documented and responded to appropriately, and attach a copy of the alarm history with <br />documentation taken in response to any alarms to the monthly report. Ensure that designated operators <br />performing monthly inspections at this facility are including all of the required information on the reports. <br />303. The diesel turbine sump sensor was raised approximately five inches off the lowest point of the <br />sump and not located to detect a leak at the earliest opportunity. Monitoring equipment shall be <br />maintained to be able to detect a leak at the earliest possible opportunity. During the inspection the <br />technician repositioned the sensor to the lowest point of the sump. Corrected on site. <br />305. The 91 Bravo box float and chain in the #7/8 sump failed to stop the flow of product at the <br />dispenser when tested. All monitoring equipment shall be maintained to activate an audible and visual <br />alarm or stop the flow of product at the dispenser when it detects a leak. The service technician <br />adjusted the chain and verified functionality. <br />305. The 87, 91, and diesel Bravo box float and chain in the #5/6 sump were submerged underneath <br />approximately four gallons of diesel and did not trip the shear valve. All monitoring equipment shall be <br />maintained to activate an audible and visual alarm or stop the flow of product at the dispenser when it <br />detects a leak. The service technician removed the diesel fuel, adjusted the 87 chain and verified <br />functionality of the 87, 91, and diesel Bravo box float and chains. The removed diesel was returned to <br />the diesel tank. Corrected on site. <br />306. A fuel leak was found at the #5/6 dispenser diesel filter. All primary containment must be product <br />tight. The technician tightened the diesel filter to stop the leak. Corrected on site. <br />310. Approximately one pint of liquid was observed in the diesel turbine sump. If water could enter into <br />the secondary containment by precipitation or infiltration, it must be removed and disposed of properly. <br />Immediately remove this liquid, make a hazardous waste determination per Title 22 hazardous waste <br />regulations and manage it accordingly.
