Kettleman Ln. Lodi, CA 95240 <br />Program: UST <br />SUMMARY OF VIOLATIONS <br />CLASS I, CLASS II, or MINOR -Notice to Comply) <br />#21 One gallon of water was observed in the diesel stp sump. A half gallon of water was observed in the <br />87#1 (West tank) stp sump. One quart of water was observed in the 91 stp sump. Two gallons of water <br />was observed in the 87#2(East tank) fill sump. Two gallons of water was observed in the 87#1 (West <br />tank) fill sump. Two gallons of water was observed in the in the diesel fill sump. If water could enter into <br />the secondary containment by precipitation or infiltration, it must be removed and disposed of properly. <br />Technician corrected this during the inspection by removing the liquid and placed it into the hazardous <br />waste drum on site. Ensure that the above sumps are maintained free of liquid. <br />#23c The designated operator failed to document all the alarms in the previous ten months and check <br />that they were responded to appropriately on the August 2009, September 2009, October 2009, and <br />June 2010 designated operator monthly inspection reports. The missing alarms include: Q1 91 PLLD <br />shutdown alarms on June 2 & 4, 2010 and October 20, 2009; Q2 87#2(East tank) PLLD shutdown <br />alarms on October 20, 2009, September 4, 2009, and August 20, 2009; and Q3 87#1 (West tank) PLLD <br />shutdown alarms on October 2 & 20, 2009. The August 18, 2009 D.O. report did not have attached the <br />Q alarm history report. During the monthly inspection, the designated operator shall review the alarm <br />history for the previous month, check that each alarm was documented and responded to appropriately, <br />and attach a copy of the alarm history with documentation taken in response to any alarms to the <br />monthly report. Ensure that designated operators performing monthly inspections at this facility are <br />including all of the required information on the reports. <br />#24 Annual monitoring system certification, leak detector testing, and spill container testing were last <br />performed on 7-9-09 and the testing of the 87#1 (West tank), 87#2(East tank), and diesel tank probe for <br />overfill prevention set at 90% was completed on 8-20-09, one month past due. These tests are required <br />once every 12 months. Take all necessary precautions to ensure testing is performed in a timely <br />manner. <br />Note: The 87#1 (West tank), 87#2(East tank) spill container testing was last performed on July 9, 2009 <br />and is due by the end of July 2010. This test is required once every 12 months. Immediately schedule <br />this test and provide 48 hours notification to the EHD. Please be aware that a follow up inspection will <br />be charged at our current hourly rate. <br />Complete and submit a copy of the Return to Compliance Certification form with a statement of how <br />each violation was corrected, including supporting documentation, by 8-26-10, 30 days from now. <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE ($116). <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT END'S CURRENT HOURLY RATE. <br />EHD Inspector: <br />Received y: <br />ITitle: <br />EHD 23-02-003 Rev 06/01/10 CONTINUATION FORM <br />
