Lodi CA 95240 SR0057929 <br />The San Joaquin County Environmental Health Department (EHD) has completed the <br />review of the addendums to the application to install CNI CON2 5 gallon spill containers <br />in the fill sump of the #4 diesel and #1 87 west tanks at the above noted address. <br />Additional scope of work includes the following: <br />1. Installing EMCO Wheaton A1100EVR-057 Overfill Prevention Valves at the 87 <br />West, 87 East, and the diesel tanks set at 95% overfill prevention. <br />2. Remove the Veeder-Root 794380-208 sump sensors from all four fill sumps. <br />3. Replace the power strip to restore functionality of the external audible overfill <br />alarm. <br />The permit has been approved with the following conditions: <br />1. Contact the local fire department and the San Joaquin Valley Air Pollution <br />Control District for any applicable permits and requirements. <br />2. Qualified service technicians must possess or work under the direct and personal <br />supervision of an individual physically present at the work site who possesses all <br />of the following: <br />a. One of several permissible valid, current contractor's licenses issued by <br />the Contractors State License Board (may be issued to the installation <br />company), and <br />b. A valid, current California UST Service Technician certificate issued by <br />the International Code Council (ICC), and <br />c. A valid, current certificate of training from the manufacturer(s) of the <br />component(s) being tested, repaired, or serviced, and <br />d. Each person installing or testing any component must hold a certificate of <br />training from the manufacturer of that component. <br />
