Note: If an extension to the <br />expiration date is required, a written request must be submitted to this office prior <br />to the expiration date. <br />4. All components must be certified by an independent testing organization and <br />labeled as required by the organization. (UL, ULC, ANSI, API, ASTM, etc. See <br />California Code of Regulations (CCR), Title 23 Chapter 16 Table B for complete <br />list). The component must bear the appropriate mark as required by the <br />independent testing organization. <br />5. Any changes in information and scope of work must be submitted to EHD for <br />approval prior to commencing work. All new addenda supersede the previous <br />ones. <br />6. Contact Aris Cacapit to arrange the following inspections (a 48 hour notice is <br />required): <br />a. One hour hydrostatic test (or equivalent) on the diesel, 87 west & east spill <br />containers. <br />b. Verify the spill containers have a minimum capacity of 5 gallons. <br />c. The test fluid should cover the top joint, and be no less than 1.5 inches <br />below the top of the spill containers, regardless of the spill container's <br />capacity. <br />d. Inspector to be present during installation of the EMCO Wheaton <br />A1100EVR-057 Overfill Prevention Valves. <br />e. Retest of the 91 overfill prevention audible visual alarm set at 90%. <br />7.
