The following items need to be addressed prior to permit approval. <br /> 1. Please submit the enclosed completed service request form to this office. <br /> 2. A written monitoring and response plan according to the requirements stated in <br /> the California Underground Storage Tank Regulations, section 2632 (d) (1) & <br /> (2) needs to be submitted to this office. <br /> 3. You need to identify the Board of Equalization number on page four of the <br /> application. <br /> 4. You need to provide the calculations to verify if the tanks will be subject to <br /> floatation. <br /> 5. The owner or operator must sign the bottom of page eight, space #25. <br /> 6. Page nine, #19 & #22, needs to be completed. <br /> 7. Please provide a detailed illustration of the piping run from the tanks to the <br /> dispenser island including secondary containment, slope, monitoring probes, <br /> etc. <br /> 8. The cross section of the tanks and equipment must represent the actual <br /> proposed construction of the tank system, not a diagram used for illustration <br /> purposes only. Please submit a cross sectional diagram of the actual <br /> proposed construction of the tank and equipment including secondary <br /> containment, overflow protection equipment, striker plates, leak detectors, and <br /> extension of all pipes and/or ports form below grade to finish grade. <br /> A Division of San Joaquin County Health Care Services <br />
