The permit has been approved with <br />the following conditions: <br />1. Contact the Fire Department and San Joaquin Valley Air Pollution Control District for <br />applicable permits. <br />2. Qualified service technicians must possess or work under the direct and personal <br />supervision of an individual physically present at the work site who possesses all of the <br />following: <br />a. One of several permissible valid, current contractor's licenses issued by the <br />Contractors State License Board (may be issued to the installation company), and <br />b. All valid, current California UST certificates issued by the International Code <br />Council (ICC), that are applicable to the work being performed, and <br />c. A valid, current certificate of training from the manufacturer(s) of the <br />component(s) being tested, repaired, or serviced. <br />and <br />d. Each person installing or testing any component must hold a certificate of training <br />from the manufacturer of that component. <br />3. All components installed must be certified by an independent testing organization and <br />bear the appropriate mark as required by the independent testing organization. <br />4. All monitoring equipment must be approved and listed in the State Water Resources <br />Control Board's Local Guidance Letter 113:"List of Leak Detection Equipment and <br />Methods for Underground Storage Tanks." <br />5. A tank integrity test must be performed by a third party certified tank tester and <br />results received by this office within 7 days of completion of work. <br />6. Contact Michelle Henry (953-7699) to arrange the following inspections appointments <br />(a 48-hour notice is required for inspection appointments): <br />a. EHD must verify the overfill prevention system is in compliance with CCR 2635 <br />(b)(2). Note: Your company may choose to have EHD onsite during the overfill <br />prevention system installation so that portions of the system do not have to be <br />disassembled/reassembled. <br />
