This <br />approval is in addition to the ISD/ EVR Phase II upgrade and cold start approved in a <br />letter dated December 12, 2007 for the above stated address. The permit has been <br />approved with the following conditions: <br />1. Contact the local fire department and the San Joaquin Valley Air Pollution <br />Control District for any applicable permits and requirements. <br />2. Contact Muniappa Naidu at (209) 468-3439 to arrange the following inspection <br />appointment (a 48-hour notice is required for inspection appointments): <br />a. Pressure and Soap test of the secondary pipe. <br />3. Any changes in information and scope of work must be submitted to EHD for <br />approval prior to commencing work. All new addenda supersede the previous <br />ones. <br />4. Individuals installing UST Systems or components shall possess or work under <br />the direct and personal supervision of an individual physically present at the work <br />site who possesses all of the following: <br />a. One of several permissible valid, current contractor's licenses issued by <br />the Contractors State License Board (may be issued to the installation <br />company), and <br />b. A valid, current International Code Council (ICC) UST <br />Installer/Retrofitting certificate and <br />c. A valid, current certificate of training from the manufacturer(s) of the <br />component(s) being installed. <br />5. Qualified service technicians must possess or work under the direct and personal <br />supervision of an individual physically present at the work site who possesses all <br />of the following: <br />
