505 N Main Street, Manteca, CA 95336 <br />Page 2 <br />a. One of several permissible valid, current contractor's licenses issued by <br />the Contractors State License Board (may be issued to the installation <br />company), and <br />b. A valid, current California UST Service Technician certificate issued by <br />the International Code Council (ICC), and <br />c. A valid, current certificate of training from the manufacturer(s) of the <br />component(s) being tested, repaired, or serviced, <br />and <br />d. Each person installing or testing any component must hold a certificate of <br />training from the manufacturer of that component. <br />6. Complete and submit a copy of a UST Installation Certificate of Compliance <br />(Former Form C) to this office no later than 30 days after work is completed. <br />7. If concrete is broken, prior to the contractor signing off of the jobsite, a tank <br />integrity test and a pipeline test must be performed by a State Water Resource <br />Control Board (SWRCB) licensed tank tester. The test results must be submitted <br />to SJCEHD within 7 days of completing this project. <br />8. Sampling will not be required unless the primary product pipe fails a line test. <br />9. All product installed must be certified by an independent testing organization and <br />labeled as required by the organization. (UL, ULC, ANSI, API, ASTM, etc. See <br />California Code of Regulations, Title 23, Chapter 16 Table B for complete list.). <br />Pursuant to the Underwriters Laboratories Inc (UL) listings, only those products <br />bearing the UL mark will be considered listed. Any part/product not marked with <br />the UL label will be rejected. <br />10. Permit expires 90 days from date of approval.
