One of several permissible valid, current contractor's licenses issued by the Contractors State License Board (may be issued to the installation company), and b. A valid, current California UST Service Technician certificate issued by the International Code Council (ICC), and c. A valid, current certificate of training from the manufacturer(s) of the component(s) being tested, repaired, or serviced, and d. Each person installing or testing any component must hold a certificate of training from the manufacturer of that component. 6. Complete and submit a copy of a UST Installation Certificate of Compliance (Former Form C) to this office no later than 30 days after work is completed. 7. If concrete is broken, prior to the contractor signing off of the jobsite, a tank integrity test and a pipeline test must be performed by a State Water Resource Control Board (SWRCB) licensed tank tester. The test results must be submitted to SJCEHD within 7 days of completing this project. 8. Sampling will not be required unless the primary product pipe fails a line test. 9. All product installed must be certified by an independent testing organization and labeled as required by the organization. (UL, ULC, ANSI, API, ASTM, etc. See California Code of Regulations, Title 23, Chapter 16 Table B for complete list.). Pursuant to the Underwriters Laboratories Inc (UL) listings, only those products bearing the UL mark will be considered listed. Any part/product not marked with the UL label will be rejected. 10. Permit expires 90 days from date of approval. Note: If an extension to the expiration date is required, a written request must be submitted to this office prior to the expiration date.
