The 89 -octane line leak detector failed to detect a leak when tested. All line leak detectors shall <br />be capable of detecting a 3 -gallon per hour leak at 10 psi. The service technician adjusted the leak <br />detector, retested it, and it passed. CORRECTED. <br />313. The 89 -octane spill container failed when tested. All spill containers shall have a minimum <br />capacity of five gallons and be capable of containing a spill or overfill until it is detected or cleaned up. <br />The technician onsite evaluated the spill container, replaced the existing fill tube cap, and retested it with <br />passing results. CORRECTED. <br />MINOR -Notice to Comply: 113 <br />113. Several new designated operators conducted the following designated operator inspections: <br />Benjamin Webb on 4-8-2014, William Culp on 5-6-2014, and "Former Employee ID" (no actual name <br />only the designated operator signature was included) on 6-3-2014. No notification was not provided to <br />the EHD within 30 days of the change. Any changes shall be reported to the EHD with 30 days. <br />Immediately, submit notification to the EHD identifying all additional designated operators for this facility. <br />Alternatively, log into CERS, upload the required information, and submit for review by the EHD. <br />Complete by July 20, 2014. <br />Complete and submit a copy of the Return to Compliance Certification form with a statement of <br />how each violation was corrected, including supporting documentation, by July 20, 2014. <br />Provided: <br />-Return to Compliance Certification Form <br />Note: Facility operators are required to upload the following UST program documents into the <br />California Environmental Reporting System (CERS): UST Monitoring Site Plan, UST Certification of <br />Financial Responsibility, UST Response Plan, UST Owner/Operator: Written Agreement (if applicable), <br />UST Letter from Chief Financial Officer, and Owner Statement of Designated UST Operator <br />Compliance. <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT END'S CURRENT HOURLY RATE. <br />EHD Inspector: <br />Received By: <br />Title: <br />EHD 23-02-003 Rev 04/19/12 CONTINUATION FORM <br />
