For the 10/12/2016. 11/7/2016, 12/1/2016,4/3/2017, 5/1/2017, and 6/1/2017 <br /> DO monthly inspection reports,the incorrect date(6/18/2017)was entered for the secondary containment testing. <br /> The correct date for that test was 8/17/2016. <br /> For the 7/5/2016 DO monthly inspection report,the incorrect date(4/20/2015)was entered for the spill container <br /> test. The correct date for that test was 6/16/2016. <br /> For the 7/5/2016, 12/1/2016,4/3/2017 and 5/1/2017 DO monthly inspection reports,the incorrect expiration date <br /> was entered for the Curtis Carpenter's DO ICC license expiration date. The 7/5/2016 DO report shows that the DO <br /> license expired on 6/16/2015 and the 12/1/2016 DO report shows that the DO license expired on 3/20/2015. The <br /> 4/3/2017 and the 5/1/2017 DO inspection report shows that the DO license expired on 2/5/2019 when the correct <br /> date is 2/6/2019. <br /> During the monthly inspection,the designated operator shall check that all required testing and maintenance for the <br /> UST system have been completed and document the dates they were done. Ensure that designated operators <br /> performing monthly inspections at this facility are including all of the required information on the reports. <br /> This is a minor violation. <br /> 211 CCR 2715(f) Designated operator employee training not performed or log not kept. <br /> The designated operator employee training log for Jass Singh did not not include the hiring date. According to Sat <br /> Singh(manager),Jass was hired after July 1,2005. After July 1, 2005,the hiring dates need to be included with the <br /> employee training date. <br /> Ensure that employees have been trained by the designated operator, maintain the list on site, and include the hiring <br /> dates. <br /> This was corrected on site. <br /> This is a repeat violation, Class II. <br /> 309 CCR 2636(f)(1)Monitoring in sump/UDC failed to activate an audible and visual alarm or stop the flow product. <br /> The Bravo Float and Chain for the 87 product in the 1/2 under dispenser containment unit(UDC)sump failed to stop <br /> the flow of product at the dispenser when tested.
