Yosemite Ave. Manteca, CA 95336 Program: UST SUMMARY OF VIOLATIONS CLASS I, CLASS 11, or MINOR -Notice to Comp On site to conduct the routine inspection and witness the annual monitoring system certification. Violations: 3, 4, 5, 17, 21, 23a, 23c. Repeat Violations- 7, 24. Violations-. #3 UST facility and tank forms on file with the EHD are not current. Any change of information on these forms must be submitted to the EHD within 30 days of the changes. Immediately complete and submit a copy of the facility and tank forms to the EHD. Complete a separate tank form for each tank. Note: Forms provided. #4 Financial responsibility documents have not been submitted to the EHD. Current financial responsibility documents are required to be submitted annually. Complete and submit a copy of the financial responsibility by 8-20-10. Note- Forms provided. #5 The response plan on site is not current. The response plan must be current and approved by the EHD. Complete and submit a copy of the response plan for approval by the EHD by 8-20-10. An approved copy will be returned for your records. Note: Form provided. #17 The 87 line leak detector failed to detect a leak when tested. All line leak detectors shall be capable of detecting a 3 -gallon per hour leak at 10 psi. The service technician adjusted the leak detector, retested it, and it passed. #23a A new designated operator was hired on May 2008 and notification was not provided to the EHD within 30 days of the change. Any changes shall be reported to the EHD with 30 days. Submit notification to the EHD identifying all the designated operators for this facility by 8-20-10. Note-. Owner Statements of Designated UST Operator and Understanding of and Compliance w/ UST requirements form provided. #23c The designated operator failed to document all the alarms for the previous month on the Sep. 2009 and Feb. 2010 designated operator monthly inspection reports. The missing alarms include- L1: Disp. 1- 2 sensor out on 9-19-09 and 1-31-10. During the monthly inspection, the designated operator shall review the alarm history for the previous month, check that each alarm was documented and responded to appropriately, and attach a copy of the alarm history with documentation taken in response to any alarms to the monthly report. Ensure that designated operators performing monthly inspections at this facility are including all of the required information on the reports. ALL EHD STAFF TOLE ASSOCIATED WITH FAILING To coYPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE ($115).
