The wrong financial responsibility mechanism was selected. <br /> Any change of information must be updated in CERS within 30 days of the changes. Immediately log into CERS, <br /> update the required information, and submit for review by the EHD. <br /> UST Tank Information forms for the 87 and 91 tanks are not current in CERS. The submittal was restarted from <br /> scratch and the information placed in the most current tank forms is not accurate. Any change of information must <br /> be updated in CERS within 30 days of the changes. Immediately log into CERS, update the required information, <br /> and submit for review by the EHD. <br /> This is a repeat violation, Class II. <br /> 106 CCR 2632 Monitoring and response plans not current or approved by the EHD. <br /> The monitoring plan is not current and/or not approved by the EHD. The most recent monitoring plans submitted on <br /> CERS are blank. The monitoring plan must be uploaded to the California Environmental Reporting System (CERS). <br /> Immediately log into CERS, make the necessary changes, and submit for review by the EHD. <br /> This is a minor violation. <br /> 107 CCR 2632(d)(1)(C) Plot plan/site map not submitted or failed to completely show where monitoring is performed. <br /> An accurate UST Monitoring Site Plan was not submitted. The site map submitted was for 713 N. EI Dorado St., <br /> Stockton. A site plan must be submitted identifying the locations where monitoring will be performed. Immediately <br /> log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/and upload a copy of <br /> the UST Monitoring Site Plan. <br /> This is a repeat violation, Class II. <br /> 201 CCR 2712(b) Failed to maintain alarm logs and/or records of follow up actions. <br /> Maintenance and monitoring records for the last three years were not found on site. The test results for the <br /> monitoring system certification and the secondary containment testing were not found on site. These records shall <br /> be maintained on site for at least three years.
